By avoiding the rectal wall, transperineal access substantially lowers post‑procedure infection compared with the transrectal route, shifting the fundamental risk profile of prostate sampling. That shift aligns the procedure with local anaesthesia rather than routine general or IV sedation, cutting staffing and recovery requirements.

In a single‑centre cohort of 34 patients undergoing transperineal prostate biopsy, patients reported a mean pain score of 4.9, indicating moderate discomfort that remains within a pragmatic range for an office‑based procedure under local anaesthesia.

Local anaesthesia removes the need for operating‑room time, anaesthetic staff, and extended recovery while shortening total procedure time and still permitting adequate tissue sampling for diagnosis. The approach supports same‑day ambulatory workflows and simpler scheduling across frontline urology services—changes that can improve patient access and service efficiency while preserving diagnostic goals.
